Step One

If your account has been locked or you have forgotten your password, you can have it reset from this page. In order to proceed, you must have access to a telephone with a direct dial number or can receive text messages. After providing the information below, our system will call you at the telephone number you enter below or send you a text message to provide you with a reset code. You must enter that code on the next page along with your new password to have it reset.

Don't know your participant ID?

Your unique Participant ID number was mailed to your home address when you signed up for the plan. The PID can also be found on:

  • Rejection Notice – your PID is located in the upper right hand corner below the bar code.
  • Reimbursements – Your PID appears on you reimbursement check stubs or direct deposit confirmations.
  • Claim Forms – if your claim form is pre-filled in with your information, your PID is located directly below your Group Name.
  • E-mail – many of our system emails will list your PID number towards the top of the document.
  • Human Resources Department – your HR department may have this number on file.
